COMMUNITY CARE

SERVICES PROGRAM

The Community Care Services Program (CCSP) assists individuals who are elderly and/or
functionally impaired to remain in their homes and/or communities. For eligible consumers, CCSP
offers community-based services as an alternative to nursing facility placement.

What services are available through this program?

 Adult Day Health  Personal Support Services
Daytime care and supervision in an adult day center Assistance with meal preparation, bathing, and light house
 Alternative Living Services keeping

Alternative residence for persons unable to remain Respite Care

independent in their own homes  Consumer-Directed Personal Support Services
 Emergency Response Services Consumer hires and supervises worker(s) of choice
In-home electronic support system providing two-way  Out-of-Home Respite Care
communication between isolated persons and a medical Out-of-home overnight respite care in an approved facility
control center with 24-hour supervision
 Home-Delivered Services  Home Delivered Meals
Skilled nursing services, as well as physical, speech and Prepared outside the home and delivered to the client
occupational therapy
Who is eligible for this program?

The eligibility criteria for CCSP include the following:
Physically impaired
In need of care

Medicaid eligible

CSRA Area Agency on Aging
